In an interview with Bill Withers, he told how this song came about: "I was watching a movie called Days Of Wine And Roses (1962) with Lee Remick and Jack Lemmon. They were both alcoholics who were alternately weak and strong. It's like going back for seconds on rat poison. Sometimes you miss things that weren't particularly good for you. It's just something that crossed my mind from watching that movie, and probably something else that happened in my life that I'm not aware of."

Orbison claimed to have written this as the result of an encounter he had with an old flame with whom he was still in love. He refused to say how much she meant to him, and when he ran into her again it was too late. At the time, rock artists didn't typically write songs about crying over a girl. Orbison wanted to show that crying was not weakness, but sensitivity.

Smith says that after reading it, he destroyed his old personal photos and many of his home videos in an effort to wipe away his past. He came to regret the decision a few days later.

The video was directed by Mark Romanek, who had directed the clips for Madonna's "Rain" and Lenny Kravitz' "Are You Gonna Go My Way." Romanek says that the video for "Closer" was shot on a hand-cranked camera from 1919, and was made "for art's sake." Like the song, the video is raw and disturbing, complete with bondage, nudity, and a monkey strapped to a crucifix. When MTV showed interest, they edited it for broadcast and the video went in rotation on the network. As for the monkey, Romanek insists that it was never harmed, and that an ASPCA representative was on the set to ensure its safety.
